Text messaging terms

These terms describe optional patient text messages from Resolution Health (Dawnbreaker Health LLC), delivered through Hero EMR.
Messages and consent
If you choose to enroll, messages may include appointment confirmations and reminders, scheduling updates, patient portal invitations, and billing reminders. This program is not for marketing or promotional messages.
Ask practice staff to explain the disclosure and record your explicit consent in person or by phone before enabling texts. Consent is optional and is not a condition of care. Providing your mobile number on an appointment or waitlist form does not enroll you.
Frequency and carrier charges
Message frequency varies based on your appointments and communication needs. Message and data rates may apply according to your carrier plan.
STOP and HELP
Reply STOP to any message to opt out at any time. You may receive a confirmation of your opt-out. You can also ask practice staff to stop texts. Opting out does not affect your care.

Privacy
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Your mobile number and consent are used to provide the messages you requested. Service providers may process this information to deliver and support those messages. Text-message opt-in data and consent are not shared for other purposes. See our privacy policy.
Delivery and medical information
Delivery depends on your carrier and network availability. Carriers are not liable for delayed or undelivered messages. Do not send personal health information by text. Use the patient portal for medical information. Text messages are not for emergencies; call 911 for urgent or life-threatening concerns.
