Information you choose to provide
Appointment, office-question, and Medicare waitlist forms collect your name, phone number, email, and the scheduling details you choose to provide. Insurance plan name and a short message are optional. Please do not include symptoms, diagnoses, insurance member numbers, payment information, or other medical details.
How your information is used
Form submissions are sent to the practice through Hero EMR to respond to your request, discuss scheduling, or follow up about the Medicare waitlist. Use the patient portal for medical information. The website forms do not book an appointment.
Website operation
Website and form service providers process technical information needed to deliver and protect these services, such as network addresses and request logs. These pages do not use advertising trackers or store form entries in your browser’s local storage. If bot verification is required, Google reCAPTCHA is loaded to help protect the form from abuse.
Text messaging privacy
Providing a phone number alone does not enroll you in text messages. If you opt in separately, your mobile number, consent records, delivery records, and replies are used to deliver the messages you requested and honor opt-out and help requests.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Service providers may process your number and consent information only as needed to deliver messages and support the service on the practice’s behalf. Text-message opt-in data and consent are not shared for other purposes. We do not sell your mobile number.
